Consent for Use of Personal Information:

I authorize the Canadian Soccer Association, the Ontario Soccer Association, The Peel Halton Soccer Association, and my Club to collect and use personal information about me or my child/ward for the purpose of receiving communications; and the disclosure of my child/ward’s name and address to the (Municipality) for the purpose of securing fields and no other purpose. I understand that I may withdraw such consent related to receiving communications at any time by contacting the OSA Privacy Officer at OSAPrivacyOfficer@soccer.on.ca or by mail to: Attention: OSA Privacy Officer, OSA, 7601 Martin Grove Road, Vaughan ON L4L 9E4. The Privacy Officer will advise the implications of such withdrawal.

Acceptance of Terms and Conditions:

In consideration of the acceptance of my or my child/ward’s membership in the Ontario Soccer Association, District Association and Club, I, the participant and parent/guardian (if participant is under 18 years of age), agree as follows:

  1. I understand that my child/ward cannot play in any sanctioned soccer game until after this registration form has been validated and the registration data has been entered in The Ontario Soccer Association’s computerized registration system.
  2. I have reviewed the waiver/participation agreement attached & my signature affixed hereto indicates my agreement with such waiver/participation agreement.
  3. I am aware of The Ontario Soccer Association, The Peel Halton Soccer Association, my Club and League bylaws, policies, rules and regulations and agree to abide by them and to be bound by them.
  4. I accept sole responsibility for me or my child/ward’s personal possessions and athletic equipment.
  5. I accept all liability for any damage to the playing equipment caused by me, or my child/ward’s careless, negligent and/or improper handling. I acknowledge that I have read this registration agreement in its entirety and that I have executed this registration agreement voluntarily.
  6. I agree that team pictures that have my child may be used on the ISC social media and website and not for any other purposes.

Refund Policy:

  • All payments are fully reimbursed by way of credit if the Club cannot host your age group.
  • All refunds are subject to $85 administration fee. No refunds will be granted after February 1st for March Break Camp or after April 1st for the outdoor season or after October 1st for the indoor season.
  • Requests for refund must be made in writing to Info@internationalsoccerclub.ca. Please allow 4 weeks for a refund cheque to be processed.
  • In case of season-ending injury or illness, such refunds may come in the form of credit but subject to the approval of the Club Registrar.
  • No refunds shall be granted to Rep, Rep Development, High Performance, All Star or Select Players.
  • I agree that failure to adhere to the Club Policies, Rules and Regulations that are currently posted on the Club website can jeopardize my membership and I may face consequences that include, but not limited to, termination of my enrollment in the program.

Ontario Soccer Association Waiver of Liability:

IN CONSIDERATION of allowing my minor child/ward to participate in the programs, activities and events of The OSA, I ASSURE TO YOU THAT:

    1. I am the parent/guardian of the above named participant having full legal responsibility for decisions regarding the above named participant.
    2.  I believe that my minor/ward is physically, emotionally and mentally able to participate in the programs, activities and events of The OSA.
    3. I hereby acknowledge that I am aware of the risks and hazards associated with or related to soccer. The risks and hazards include, but are not limited to injuries from:
      1. Executing strenuous and demanding physical techniques in soccer;
      2. Dry land training including weights, running and massage;
      3. Grass, turf and other surfaces including bacterial infections and rashes;
      4. Falls to the ground due to uneven or irregular terrain or surfaces;
      5. Collisions with walls and soccer equipment;
      6. Failure to properly use any piece of equipment or from the mechanical failure of any piece of equipment;
      7. Extreme weather conditions which may result in heatstroke, sunstroke or hypothermia;
      8. Contact, colliding or being struck by other participants, spectators, equipment or vehicles;
      9. Vigorous physical exertion and strenuous cardiovascular workouts;
      10. Exerting and stretching various muscle groups;
      11. Travel to and from competitive events and associated non-competitive events which are an integral part of the organization’s activities.
    4. Furthermore, I am aware that my child/ward may:
      1. Sustain injuries in soccer that can be severe, cause spinal cord injuries and even be fatal;
      2. Experience anxiety while challenging himself/herself during the activities, events and programs;
      3. Come into close contact with other participants, including the possibility of accidental and unexpected contact;
      4. Risk of injury is reduced if he/she follows all rules established for participation;
      5. Risk of injury increases as he/she become fatigued.

I UNDERSTAND AND AGREE, on behalf of myself, my heirs, assigns, personal representatives and next of kin that my signing of this document constitutes:

  1. I am registering my child/ward willingly and my child/ward is participating voluntarily in these activities, events and programs.
  2. I agree that there are risks in soccer as described above and my child/ward will be exposed to these risks and hazards.
  3. I agree to accept all these risks and hazards and be responsible for any injury or other loss which my minor child/ward might receive while participating in these events, activities and programs.
  4. If something happens to my child/ward, I release the Organizers of responsibility for any claims, demands, actions and costs which might arise out of my child/ward’s participation. I understand “Organizers” to mean: The Ontario Soccer Association, District Associations, Leagues, Clubs and their directors, officers, members, employees, volunteers, officials, participants, clubs, agents, sponsors, owners/operators of facilities, and representatives.

Accident Insurance: Executing this agreement will not preclude you from accident insurance coverage, subject to the terms and conditions of The Ontario Soccer Association’s insurance policy. I ACKNOWLEDGE MAKING THIS AGREEMENT I have read and understood the terms and conditions of this agreement, and by signing it voluntarily on the printable version of this form, I am agreeing to abide by these terms.

Ontario Soccer Covid-19 Waiver of Liability:

WARNING ! ALL INDIVIDUALS ENTERING THE FACILITY MUST COMPLY WITH THIS DECLARATION Ontario Soccer and its affiliated districts, leagues, clubs and teams (collectively the “Organization”) require the disclosure of exposure or illness is in order to safeguard the health and safety of all participants and limit the further outbreak of COVID-19. This Declaration of Compliance will be kept safely, and personal information will not be disclosed unless as required by law or with your consent. An individual (or the individual’s parent/guardian, if the individual is younger than 18 years old) who is unable to agree to the terms outlined in this document is not permitted to enter the Organization’s facilities or participate in the Organization’s activities, programs, or services. I, the undersigned being the individual named above and the individual’s parent/guardian (if the individual is younger than 18 years old), hereby acknowledge and agree to the terms outlined in this document:

  • The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization and COVID-19 is extremely contagious. The Organization has put in place preventative measures to reduce the spread of COVID-19 and requires all individuals (or their parent/guardian, when applicable) to adhere to the compliance standards described in this document.
  • The individual has not been diagnosed with COVID-19. OR If the individual was diagnosed with COVID-19, the individual was cleared as noncontagious by provincial or local public health authorities.
  • The individual has not been exposed to a person with a confirmed or suspected case of COVID-19. OR If the individual was exposed to a person with a confirmed or suspected case of COVID-19, the date of exposure was more than 14 days prior to the date this Declaration of Compliance was signed.
  • The individual is attending or participating voluntarily and understands the risks associated with COVID-19. The individual (or the individual’s parent/guardian, on behalf of the individual (when applicable)) agrees to assume those risks, including but not limited to exposure and being infected.
  • The individual has not, nor has anyone in the individual’s household, experienced any signs or symptoms of COVID-19 in the last 14 days (including fever, new or worsening cough, fatigue, chills and body aches, respiratory illness, difficulty breathing, nausea, vomiting or diarrhea, pink eye, or loss of taste or smell).
  • If the individual experiences, or if anyone in the individual’s household experiences, any signs or symptoms of COVID-19 after submitting this Declaration of Compliance, the individual will immediately isolate, notify the Organization, and not attend any of the Organization’s facilities, activities, programs or services until at least 14 days have passed since those symptoms were last experienced.
  • The individual has not, nor has any member of the individual’s household, travelled to or had a lay-over in any country outside Canada in the past 14 days. If the individual travels, or if anyone in the individual’s household travels, after submitting this Declaration of Compliance, the individual will not attend any of the Organization’s facilities, activities, programs or services until at least 14 days have passed since the date of return.
  • The individual is following recommended guidelines, including but not limited to practicing physical distancing, trying to maintain separation of six feet from others, adhering to recognized hygiene best practices, and otherwise limiting exposure to COVID-19.
  • The individual will follow the safety, physical distancing and hygiene protocols of the Organization.
  • This document will remain in effect until the Organization, per the direction of the provincial government and provincial and local public health authorities, determines that the acknowledgements in this Declaration of Compliance are no longer required.
  • The Organization may remove the individual from the facility or from participation in the activities, programs or services of the Organization at any time and for any reason if the Organization believes, in its sole discretion, that the individual is no longer in compliance with any of the standards described in this document.

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT (To be signed by Participants of the age of majority and over) WARNING! By signing this document, you will waive certain legal rights. Please read carefully.

  1. This is a binding legal Clarify any questions or concerns before signing. As a Participant in activities, programs, classes, services provided and events sponsored or organized by Ontario Soccer and its affiliated districts, leagues, clubs and teams and the sport of soccer, including but not limited to: games, tournaments, practices, training, personal training, dry land training, use of strength training and fitness conditioning equipment, machines and facilities, nutritional and dietary programs, orientational or instructional sessions or lessons, aerobic and anaerobic conditioning programs (collectively the “Activities”), the undersigned acknowledges and agrees to the following terms outlined in this agreement:

Disclaimer

  1. Ontario Soccer, its affiliated districts, leagues, clubs and teams, and their respective directors, officers, committee members, members, employees, coaches, volunteers, officials, participants, agents, independent contractors, subcontractors, sponsors, owners/operators of the facility in which the Activities take place, , successors and assigns, and representatives (the “Organization”) are not responsible for any injury, property damage, expense, loss of income, damage or loss of any kind suffered by a Participant during, or as a result of, the Activities, caused in any manner whatsoever including, but not limited to, the negligence of the Organization. I have read and agree to be bound by paragraphs 1 and 2
  1.  Description of Risks I understand and acknowledge that
  2. The Activities have foreseeable and unforeseeable inherent risks, hazards and dangers that no amount of care, caution or expertise can eliminate, including without limitation, the potential for serious bodily injury, permanent disability, paralysis and loss of life;
  3. The Organization may offer or promote online programming (such as webinars, remote conferences, workshops, and online training) which have different foreseeable and unforeseeable risks than in-person programming;
  4. The Organization has a difficult task to ensure safety and it is not infallible. The Organization may be unaware of my fitness or abilities, may misjudge weather or environmental conditions, may give incomplete warnings or instructions, and the equipment being used might malfunction; and
  5. The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization and COVID-19 is extremely contagious. The Organization has put in place preventative measures to reduce the spread of COVID-19; however, the Organization cannot guarantee that the Participant will not become infected with COVID-19.  Further, attending the Activities could increase your risk of contracting COVID-19.
  1. I am participating voluntarily in the Activities. In consideration of my participation, I hereby acknowledge that I am aware of the risks, dangers and hazards associated with or related to the Activities which could result in damage, loss, serious physical injury, or death. The risks, dangers and hazards include, but are not limited to, injuries from:
    1. Health: executing strenuous and demanding physical techniques, physical exertion, overexertion, stretching, dehydration, fatigue, cardiovascular workouts, rapid movements and stops, lack of fitness or conditioning, traumatic injury, bacterial infections, rashes, and the transmission of communicable diseases, including viruses of all kinds, COVID-19, bacteria, parasites or other organisms or any mutation
    2. Premises: defective, dangerous or unsafe condition of the facilities; falls; collisions with objects, walls, equipment or persons; dangerous, unsafe, or irregular conditions on grass, turf or other surfaces, extreme weather conditions; travel to and from premises
    3. Use of Equipment: mechanical failure of the equipment; negligent design or manufacture of the equipment; the provision of or the failure by the Organization to provide any warnings, directions, instructions or guidance as to the use of the equipment; failure to use or operate the equipment within my own ability.
    4. Contact: contact with soccer balls, other equipment, poles, stands, soccer equipment, nets, fences, or other persons, whether intentional or unintentional, is a common part of soccer programs, and may lead to serious bodily injury, including but not limited to concussions and/or other brain injury, or serious spinal
    5. Advice: negligent advice regarding soccer programs
    6. Ability: Failing to act safely or within my own ability or within designated areas
    7. Sport: the game of soccer and its inherent risks
    8. Conduct: My conduct and conduct of other persons including any physical altercation between soccer participants
    9. Travel: Travel to and from the Activities
    10. Negligence: My negligence and negligence of other persons, including NEGLIGENCE ON the PART OF THE ORGANIZATION, may increase the risk of damage, loss, personal injury or death. I understand that the Organization may fail to safeguard or protect me from the risks, dangers and hazards of soccer programs, some of which are referred to

Terms

  1. In consideration of the Organization allowing me to participate in the Activities, I agree:
  2. That my mental and physical condition is appropriate to participate in the Activities and I assume all risks related to my mental or physical condition;
  3. That when I practice or train in my own space, I am responsible for my surroundings and the location and equipment that I select;
  4. To comply with the rules and regulations for participation in the Activities;
  5. To comply with the rules of the facility or equipment;
  6. That if I observe an unusual significant hazard or risk, I will remove myself from participation and bring such to the attention of a The Organization representative immediately;
  7. The risks associated with the Activities are increased when I am impaired, and I agree not to participate if impaired in any way;
  8. That it is my sole responsibility to assess whether any Activities are too difficult for me. By commencing an Activity, I acknowledge and accept the suitability and conditions of the Activity;
  9. That I am responsible for my choice of protective equipment and the secure fitting of that equipment; and
  10. That COVID-19 is contagious in nature and I may be exposed to or infected by COVID-19 and such exposure may result in personal injury, illness, permanent disability or death and voluntarily agree to assume all the foregoing risks.

I have read and agree to be bound by paragraphs 3 – 5. Release of Liability and Disclaimer

  1. In consideration of the Organization allowing me to participate in the Activities, use its equipment and facilities, I agree:
  2. That the sole responsibility for my safety remains with me;
  3. To ASSUME all risks arising out of, associated with or related to my participation;
  4. That I am not relying on any oral or written statements made by the Organization or its agents, whether in a brochure or advertisement or in individual conversations, to agree to be involved in the Activities;
  5. To WAIVE any and all claims that I may have now or in the future against the Organization;
  6. To freely ACCEPT AND FULLY ASSUME all such risks and possibility of personal injury, death, property damage, expense and related loss, including loss of income, resulting from my participation in the activities, events and programs of the Organization;
  7. To FOREVER RELEASE AND INDEMNIFY the Organization from any and all liability for any and all claims, demands, actions, damages (including direct, indirect, special and/or consequential), losses, actions, judgments, and costs (including legal fees) (collectively, the “Claims”) which I have or may have in the future, that might arise out of, result from, or relate to my participation in the Activities, even though such Claims may have been caused by any manner whatsoever, including but not limited to, the negligence, gross negligence, negligent rescue, omissions, carelessness, breach of warranty, breach of contract and/or breach of any statutory duty of care of the Organization;
  8. TO HOLD HARMLESS AND INDEMNIFY the Organization from any and all liability for any damage, loss, expense or injury to any third party resulting from my participation in the Activities.
  9. To FOREVER RELEASE AND INDEMNIFY the Organization relating to becoming exposed to or infected by COVID-19 which may result from the actions, omission or negligence of myself and others, including but not limited to the Organization;
  10. That the Organization is not responsible or liable for any damage to my vehicle, property, or equipment that may occur as a result of the Activities;
  11. That negligence includes failure on the part of the Organization to take reasonable steps to safeguard or protect me from the risks, dangers and hazards associated with the Activities; and
  12. This release, waiver and indemnity is intended to be as broad and inclusive as is permitted by law of the Province of Ontario and if any portion thereof is held invalid, the balance shall, notwithstanding, continue in full legal force and effect.

Jurisdiction

  1. I agree that in the event that I file a lawsuit against the Organization, I agree to do so solely in the province of Ontario, Canada and further agree that the substantive law of Ontario will apply without regard to conflict of law rules. I further agree that if any portion of this agreement is found to be void or unenforceable, the remaining document shall remain in full force and effect.

I have read and agree to be bound by paragraphs 6 – 7  Acknowledgement

  1. I acknowledge that I have read and understand this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, spouse, children, parents, guardians, next of kin, executors, administrators and legal or personal representatives. I further acknowledge by signing this agreement I have waived my right to maintain a lawsuit against the Organization on the basis of any claims from which I have released herein.

INFORMED CONSENT AND ASSUMPTION OF RISK AGREEMENT (To be executed by Participants under the age of majority) WARNING! By signing this document, you will assume certain risk and responsibilities.  Please read carefully!

As a Participant in activities, programs, classes, services provided and events sponsored or organized by Ontario Soccer and its affiliated districts, leagues, clubs and teams (collectively the “Organization”) and the sport of soccer, including but not limited to: games, tournaments, practices, training, personal training, dry land training, use of strength training and fitness conditioning equipment, machines and facilities, nutritional and dietary programs, orientational or instructional sessions or lessons, aerobic and anaerobic conditioning programs (collectively the “Activities”), the undersigned being the Participant and Participant’s Parent/Guardian (collectively the “Parties”) acknowledges and agrees to the following terms outlined in this agreement:
  2. I am the Parent/Guardian of the Participant and have full legal responsibility for the decisions of the Participant.

 Description of Risks

  1. The Parties understand and acknowledge that:

  1. The Participant is participating voluntarily in the Activities. In consideration of that participation, the Parties hereby acknowledge that they are aware of the risks, dangers and hazards and may be exposed to such risks, dangers and hazards. The risks, dangers and hazards include, but are not limited to:
  2. Contracting COVID-19 or any other contagious disease;
  3. Executing strenuous and demanding physical techniques;
  4. Vigorous physical exertion, strenuous cardiovascular workouts and rapid movements;
  5. Exerting and stretching various muscle groups;
  6. The failure to properly use any piece of equipment or from the mechanical failure of any piece of equipment;
  7. Spinal cord injuries which may render the Participant permanently paralyzed;
  8. Serious injury to virtually all bones, joints, ligaments, muscles, tendons and other aspects of the Participant’s body or to the Participant’s general health and well-being;
  9. Abrasions, sprains, strains, fractures, or dislocations;
  10. Concussion or other head injuries, including but not limited to, closed head injury or blunt head trauma;
  11. Physical contact with other participants, spectators, equipment, and hazards;
  12. Not wearing appropriate safety or protective equipment, such a helmet;
  13. Failure to act safely or within the Participant’s ability or within designated areas;
  14. Grass, turf, and other surfaces including bacterial infections and rashes;
  15. Collisions with fences, poles, stands, and soccer equipment;
  16. Negligence of other persons, including other spectators, participants, or employees;
  17. Weather conditions; and
  18. Travel to and from competitive events and associated non-competitive events which are an integral part of the Activities

We have read and agree to be bound by paragraphs 1 – 4 Terms

  1. In consideration of the Organization allowing the Participant to participate in the Activities, the Parties agree:
  2. That the Participant’s mental and physical condition is appropriate to participate in the Activities;
  3. That when the Participant practices or train in his or her own space, the Parties are responsible for the Participant’s surroundings and the location and equipment that is selected for the Participant;
  4. To comply with the rules and regulations for participation in the Activities;
  5. To comply with the rules of the facility or equipment;
  6. That if the Participant observes an unusual significant hazard or risk, the Participant will remove themselves from participation and bring such to the attention of an Organization representative immediately;
  7. The risks associated with the Activities are increased when the Participant is impaired and the Participant agrees not to participate if impaired in any way;
  8. That it is their sole responsibility to assess whether any Activities are too difficult for the Participant. By the Participant commencing an Activity, they acknowledge and accept the suitability and conditions of the Activity;
  9. That they are responsible for the choice of the Participant’s protective equipment and the secure fitting of the protective equipment;
  10. That COVID-19 is contagious in nature and the Participant may be exposed to or infected by COVID-19 and such exposure may result in personal injury, illness, permanent disability or death and voluntarily agree to assume all of the foregoing risks.

 

  1. In consideration of the Organization allowing the Participant to participate, the Parties agree:
  2. That the Parties are not relying on any oral or written statements made by the Organization or their agents, whether in brochure or advertisement or in individual conversations, to agree to be involved in the Activities; and
  3. That the Organization is not responsible or liable for any damage to the Participant’s vehicle, property, or equipment that may occur as a result of the Activities.

We have read and agree to be bound by paragraphs 5 – 6 General

  1. The Parties agree that in the event that they file a lawsuit against the Organization, they agree to do so solely in the province of Ontario, Canada and they further agree that the substantive law of Ontario will apply without regard to conflict of law rules.
  2. The Parties expressly agree that this Agreement is intended to be as broad and inclusive as is permitted by law and that if any of its provisions are held to be invalid, the balance shall, notwithstanding, continue in full legal force and effect.

Acknowledgement

  1. The Parties acknowledge that they have read this agreement and understand it, that they have executed this agreement voluntarily, and that this Agreement is to be binding upon themselves, their heirs, their spouses, parents, guardians, next of kin, executors, administrators and legal or personal representatives.
